Questionnaire

To begin the process we need to gather key information for preparing your return.  The following link will provide you a downloadable PDF form that can be sent to me pushing the "Submit" button on the last page, or you can mail the form back to me. Please note at the top of the questionnaire you need to use the drop down box to select the year you are submitting. If you are electing to do multiple years of returns we need a separate questionnaire for each year.  The PDF Version of the questionnaire requires the use of Acrobat Reader 9 or later. Once you do this, you can save the form  to your computer and call it back up as you fill in the information. If you don't have Acrobat Reader 9,version "X" is currently a free download from www.adobe.com. In order, to avoid unsuspected aggravation, you should print out copies of your work as  you go a long just in case the file fails to save or your computer has a problem <Click>--->. PDF Version
